Dr. Joshi, MD
Physician
Dr. Joshi has been practicing medicine for 40 years and has been in the Elgin area for more than 30 years. He has held leadership positions in St. Joseph and Advocate Sherman hospitals, both located in Elgin, Illinois. He has lead local PHO (Sherman Choice) later Advocate Sherman, for more than 25 years as a Medical Director.

His practice has PCMH certification as well as recertification. At Dr. Joshi's practice, every employee is considered family including patients with the same care and attention.
​
This practice provides care in prevention as a primary objective as well as treatment there after. Dr. Joshi takes care of his patients in office and at both hospitals St. Joseph and Advocate Sherman.

Practicing lifestyle/behavior modification as the first step, follow up's and medical management for goal optimization.
​
In his free time, Dr. Joshi enjoys spending time with family, playing golf, and going on road trips.

Francesca Castro, PA-C
Physician Assistant
Francesca Castro received her education from Dominican University with a Bachelor of Science in Biology/Health sciences in 2018 and a Masters in Physician Assistant studies in 2022.
​
Practicing internal medicine allows Francesca to take part in patient's care from acute to long-term needs.
​
She enjoys connecting with patients and working together to optimize their health and well being through preventative care.
​
In her free time, Francesca enjoys relaxing at home, spending time with friends and family, her dog Bucky, and loving on her new baby boy.
Jigar Dhanani
Office Manager
Divyang A. Joshi MD, PC welcomed Jigar as Office Manager in June 2017. As Manager, Jigar oversees the daily operations and complexities of a medical practice. Jigar has a focus on building relationships with everyone who comes across Dr. Joshi be it patients, other health care providers, or vendors.

Jigar graduated from DeVry University in 2012 with a Bachelors of Science Degree in Technical Management. Then in 2019, he finished his MBA from Keller Graduate School of Management along with a Health Services Management Graduate Certificate. Jigar's career has had various business and healthcare roles: Pharmacy Technician for 2+ years, Business System Analyst for 1 year, Healthcare Related Accounts Receivable Representative for 2+ years, and Office Manager for 7+ years.
​
During the work day, Jigar supports and guides his staff to deliver exceptional clinical care and customer service. Outside of the office, Jigar loves spending time with his daughter, wife, family and friends.
